Santhanam started out as a stand-up and television comedian before moving to films. His work on Lollu Sabha, where he played lead parts in parodies of mainstream Tamil productions, established him as a recognisable comic performer in Tamil entertainment.
Actor Silambarasan offered Santhanam a supporting role in Manmadhan (2004), marking his debut in Tamil cinema. He was subsequently signed on for a series of films, drawing positive notices for his performances in Sachein (2005) and Polladhavan (2007).
Santhanam appeared in a one-off leading role in Shankar's production Arai En 305-il Kadavul (2008). Around this period he became a regular comedic presence in Tamil films, and his particular brand of insult comedy gained considerable popularity. His market appeal was reported to have helped stalled films secure distribution.
He worked extensively with director M. Rajesh on a comic trilogy comprising Siva Manasula Sakthi (2009), Boss Engira Bhaskaran (2010) and Oru Kal Oru Kannadi (2012), winning the Vijay Award for Best Comedian for each of the three films. He has also collaborated repeatedly with directors A. L. Vijay, Siva and Sundar C.
His consistent box-office appeal in the early 2010s led the Tamil film industry to refer to him as the "Comedy Superstar".
In 2012, Santhanam launched a production company, Handmade Films. Its first production, Kanna Laddu Thinna Aasaiya (2013), proved commercially successful. In 2014, he played the lead role for the second time in the action comedy Vallavanukku Pullum Aayudham.
Santhanam is regarded as one of the most prominent Tamil comic actors of the late 2000s and early 2010s. His pivot from television sketch comedy to a sustained film career, his recurring collaborations with major Tamil directors, and his transition to film production with Handmade Films illustrate the trajectory of a comedian-led commercial brand in contemporary Tamil cinema.
