The origins of Osmania Medical College are rooted in the administrative and educational reforms undertaken by the Nizam's government in Hyderabad during the early twentieth century. The college was established to provide formal Western medical education to students in the Hyderabad State, at a time when modern medicine was being institutionalised across The Indian subcontinent. The institution was named after the last ruling Nizam, Mir Osman Ali Khan, who was a significant patron of education and infrastructure in the region.

Following Indian independence and the integration of Hyderabad State into The Indian Union in 1948, the college came under the administration of the Government of Andhra Pradesh. After the bifurcation of Andhra Pradesh in 2014, it became part of the newly formed state of Telangana and now functions under the Telangana state government's Department of Medical Education.

Osmania Medical College offers the Bachelor of Medicine and Bachelor of Surgery (MBBS) degree at the undergraduate level.
